This page provides links to a variety of school forms that you can open and print. All of these forms are available through our office, but if you print your own, complete them and hand them in, you can beat the rush!
The documents are in Adobe Acrobat (pdf) format; you will need the free Adobe Reader program to open them. If you need this software, it can be downloaded for free from the Adobe site.
Forms for Students and Parents
Driving Privileges ... Want to drive your own vehicle to school?
Open Campus 10 ... Open Campus privileges for grade 10
Open Campus 11/12 ... Open Campus privileges for grade 11/12
Jr. High students DO NOT have Open Campis privileges. They must bring a dated note signed by a parent each time they wish to leave school property.
FOIPP ... FOIPP form for permission to use photos
Digital Citizenship Plan ... for students using their personal electronic devices
POD Agreement ... to be signed so students can use their personal electronic devices
Work Experience Forms
Forms that must be filled out to register:
Application Form ... 1. Filled in by the student
Parent Consent Form ... 2. Filled in by a parent
Employer Form ... 3. Filled in by the employer
Other forms:
Monthly Time Sheet ... These must be signed and handed in monthly
Sample Inspection Form ...Give this to your employer at the beginning of your Work Experience
Final Evaluation Sheet ... Give this to your employer to fill in when your Work Experience is over
